In order to expedite your initial check-in at our office, please fill out our online Patient Registration form. When you submit the form, it is emailed to us through secure networks.
Patient Forms
We have made our patient forms available for downloading so you can fill them out before you arrive. Please print and review the following patient form. Keep a copy for your records and bring the signed forms (Medical Questionnaire, Signature Page and Communication Release) with you to your first appointment. We want to make your visit to Rios & Lapinski as enjoyable as possible!
